Consolidated Chapter and Note document pages into one Editor route with a dropdown to select the docType you wish to edit. Will be easier to manage as I add more doc types. Also began implementing some responsive layout elements.

Got remove link buttons to work. Got annotation links to pop up a modal! Looks like it's never gonna work with dangerouslySetInnerHTML, but I made the DraftJS link decorator a connected component and that worked.
